1.: Unrar with 7zip or Equivalent.
2.: double click: Setup-VISTA.exe or Setup-WinXP.exe   must be done  
3.: double click MakeDesktopIcon.exe and play Game from Desktop .
 extra files  .dll  you can find in ToeD folder. in case game does not start first time. 
  Special Info  
Some antivirus software with heuristics set to HIGH process, checks may 
consider the cracktro/trainer/nocd.exe as potentially dangerous and prevent
execution. This may happen to the file  Setup-VISTA.exe  or  Setup-WinXP.exe 
If this happens add the exe to a white/exclusion list at your virustool to avoid any
